What information will the appointment scheduler need when I call?

For follow-up visits, it is advisable to make that appointment before you leave the office. This will ensure that your appointment is within the time period recommended by your physician.

What should I bring with me for my appointment?

Your current insurance carrier cards, a list of your current medications and any referrals, if required by your insurance carrier.   You should arrive at least 15 minutes early for your first appointment, if you are a new patient.

What about payment?

Payment is expected at the time of your visit, including co-pays. We cannot bill you or a spouse for a co-payment.

We are off Route I-80, Exit # 308. From 80 East or West, make a right at the ramp; go to first traffic light and turn right on E. Brown St. (Texaco Station and hospital are on corners). We are on the right hand side, #239 E. Brown St.

How do I have a prescription filled or renewed?

Call 421-3872 for our prescription line. When you hear the automated message, hit #5 on your telephone. The voice automated system will guide you through the process. We cannot honor walk-ins for prescription refills. Your prescription will be called in within 48 hours.

What if I need to cancel?

Our office requires a 24-hour notice of cancellation for a scheduled appointment. There is a $35.00 fee for failure to notify the office within 24 hours.

How do I have my medical records sent to another physician? How can I obtain a copy?

All patients must sign the Authorization for Release of Health Information form and give 48 hours advance notification. If you are personally receiving the records, the charge is
$ .25/page. The release is valid for one (1) year.

What if I need assistance getting into the building?

We ask that you bring someone with you to help you into the building, as our staff is prohibited, per company policy, from engaging in activities outside of the scope of their job description.

Do you treat children?

The practice is not credentialed to see anyone under the age of 16.
